  Guests of association members invited to Reunion 2008 are requested to complete the following information form. After submitting this form you will be emailed an information form (Microsoft Word Document) to be completed and returned via USPS along with a deposit check for the association.

Hotel reservations will be separate and should only be made through a toll free number to insure proper reunion discount rate.

 All information requests and checks must be completed before October 1, 2008.
